Transportation to Daycare
Students are only allowed to ride their designated home bus unless a Request to Ride an Alternate Bus Form has been completed for DAYCARE purposes. The form is downloadable below and needs to be completed each new school year. We encourage Elementary School parents that require alternate day care for their student to print and complete the form as soon as possible. Completed forms should be forwarded to the Transportation Office at 72 Pilgrim Rd, Pembroke, MA 02359 as soon as possible to ensure proper assignment.
Forms need to be submitted every school year, information does not carry forward.
Entering information into Pick Up Patrol DOES NOT apply. The form, approval and bus assignment must be obtained through the transportation department.
